Nachrichten aus Frankreich: institutionelle Psychotherapie zwischen Kreativität und Entbindung

Abstract

Die institutionelle Psychotherapie entstand als politisch engagierte Aktion des Kampfes gegen die psychiatrische Asylpraxis. Sie stützt sich auf Grundkenntnisse der Psychoanalyse, der therapeutischen Gruppenarbeit und der interaktiven Erziehung. Nachdem sie in den 1960er Jahren zur Sektorisierung beitrug, erfuhr sie eine Spaltung in eine Gruppe psychoanalytischer Psychiater und Vertreter mit institutioneller Ausrichtung. Letztere entwickelten einen spezifischen Aufnahmemodus, eine kollektive Organisationsform, eine Vielfalt von Aktivitäten und Alternativen zur Unterbringung. Wesentlich ist die Schaffung eines Beziehungsnetzes mit direktem Bezug zum Alltag. Institutionelle Psychotherapie versucht, einen persönlichen und institutionellen Zugang jenseits der gezeigten Symptome entlang der Übertragungsbeziehungen zu entwickeln. Das Unorganisierte, die institutionellen Lücken, sind dabei ebenso behandlungsrelevant wie durchstrukturierte Zeiten und Orte. Diese integrative Behandlungsform ist durch psychiatriepolitische Maßnahmen (Bettenreduzierung, Steuerung der Verweildauer, gesundheitspolitische und nosographische Konzepte psychischer Gesundheit, Risikovermeidung, Trennung zwischen Behandlung und Beratung) störbar bzw. gefährdet, sodass der Interventionsrahmen immer wieder neu überdacht und angepasst werden muss.

The latter developed a specific recording mode, a collective form of organization, a variety of activities and alternatives for hospitalization. It is essential to establish a network of contacts directly related to everyday life. Institutional psychotherapy attempts to develop a personal and institutional access beyond the symptoms shown along the transmission relations. The unorganized, the institutional gaps are just as relevant as structured treatment times and places.
