"Das Gesundheitssystem in Deutschland ist in den letzten dreißig Jahren nicht nur Gegenstand zahlreicher Reformversuche gewesen, es sind nunmehr tatsächlich nachhaltige Veränderungen im Steuerungssystem festzustellen. Der einstmals große Einfluss der relativ autonomen gemeinsamen Selbstverwaltung ist gesunken. Durch Gesundheitsreformen intervenierte der Staat in den Regelungsbereich der Selbstverwaltung, begrenzte den Entscheidungsspielraum korporatistischer Verhandlungssysteme und implementierte ökonomische Anreize, welche das Verhalten der Leistungserbringer und Finanzierungsträger beeinflussen sollen. Die Bilanz dieser Strukturreformen für die staatliche Steuerungsfähigkeit fällt positiv aus. Reformblockaden, wie sie die Politikwissenschaft im korporatistischen System des ambulanten Sektors feststellte, wurden teilweise durchbrochen. Einerseits verbesserte der neue Steuerungsmix die Durchsetzungsfähigkeit des Staates gegenüber den korporatistischen Akteuren, andererseits scheinen die Reformen die Handlungsfähigkeit korporatistischer Institutionen zu gefährden. Im Zuge der Implementation von Wettbewerbselementen ist insbesondere das Kollektivvertragssystem politisch bedroht. Angesichts der Reformdynamik plädiert die vorliegende Arbeit für eine planvolle Modifikation korporatistischer Institutionen. Wettbewerb und ökonomische Anreize lassen erwarten, dass zur Gewährleistung der versorgungspolitischen Performanz staatliche Regulierung eher noch ausgebaut werden muss. Die Steuerungspotenziale korporatistischer Institutionen sollten erhalten bleiben, um diese staatlichen Maßnahmen zu unterstützen." (Autorenreferat)... view less

"During the last thirty years the German health care system has been subject to numerous re-form attempts, and by now some lasting important changes in the control system have been introduced. The once great influence of the relatively autonomous common self-government has decreased. Through health care reforms, the state intervened in the regulation area of the self-government, limited the deciding match space of corporatist bargaining systems, and implemented economic incentives in order to influence the behavior of providers and purchasers. As a result of these structure reforms the state is now in a better position to control. Impediments to reforms, which were noted by political scientists in the outpatient sector, were partially removed. On the one hand, the new mix of measures in the policy formulation process has strengthened the states position with regard to corporatist actors. On the other hand, the reforms seem to endanger the ability to act of corporatist institutions. The progress of competitive elements may result in a political threat to the collective bargaining system. In view of these reform dynamics the present study pleads for a well planned modification of the corporatist institutions. The spread of competition and economic incentives gives reason to expect that more state regulation is needed to guarantee socio-political standards. The resources to control of the corporatist institutions should be preserved to support these state measures." (author's abstract)... view less
